HURST EYE CARE PATIENTS
- Simply submit the completed order form below.
- We will then verify your prescription, determine if we have your contacts in stock for immediate pick-up, and contact you to advise you of the cost and make payment over the phone.
All contact lens orders must be paid for in full at the time we confirm your information. Orders for in-stock inventory may be paid for at the time you pick them up but they must be picked up within 24 hrs of your order placement or we will put them back into inventory.
- If we do not have your contacts in-stock or you require a specialty contact lens, we will provide you with an approximate date you can expect them to arrive at our office.
Typically this is around 3-5 business days, however, unforeseen circumstances such as back orders at the manufacturers can occur which may delay your order being filled. However, we will do everything within our power to ensure that your order is expedited in a timely manner.
- When your special order arrives, we will notify you.
- You may pick your contacts up at your convenience Monday thru Friday 8am to 6pm or Saturday 9am to Noon.
- For your convenience, you may request that we mail your contacts directly to you for an additional $4.95 S&H fee.
Even if you are not currently a patient of Hurst Eye Care, we would also like to assist you with your contact lens order for your convenience
- Complete and fax us the Medical Release Form (see link) so that we may contact your eye care provider to verify your prescription
- Simply submit the completed order form and follow the remaining process as listed above
